Hand Therapy Center accepts most major health insurance plans including Worker’s Compensation and Medicare. Call and confirm that we are providers for your specific insurance plan.

Before filling out the following forms please make sure you have a physician referral for Occupational Therapy/Hand Therapy as this is required by law and by your insurance company. A valid Prescription from a physician, written in the last 30 days is required.

It is the responsibility of the patient to maintain a current prescription from the referring physician.  We do give courtesy reminders 5 days prior to the expiration of your current prescription. Faxed prescriptions are acceptable and do not need to be on our prescription pads. As a courtesy we do call your insurance company to obtain therapy benefits but we strongly advise you do the same.
